3. All requests must include a fully completed and signed form along with all required documentation. You must meet the criteria, and all necessary documentation submitted, in order for your request to be processed.
4. Billing will send you an email regarding the status of your cancellation request, once it has been processed.

1. All month to month and term contract cancellations require 30 days advance notice, in which you will be billed one more time.
2. Gym does allow cancellation of term contracts, which are not expired, for the reasons below. Or you may "buy out" of the contract, meeting the clubs terms. Member must meet and be approved for one of the reasons above.
3. I understand by digitally signing this form, it is my responsibility to provide all required documentation to Ground Control. If required documentation is not provided, this request will not be processed.
4. Account must be in good standing with no outstanding dues or fees owed. In order to be eligible for cancellation all past due fees and charges must be paid in full to accept the cancellation notice.
Reason For Requesting Account Cancllation
Moved 25 or more miles from facility. (Must provide supporting documentation with request & $50.00 move fee will apply, ie: Utility Bill, Signed Lease Agreement or Mortgage Contract)
Unable to continue for Medical reasons (Must provide Physician Note to verify)
Military - For temporary relocation or Deployment (must provide military orders to verify - No Freeze Fee)
Buy Out: Buy out contract by paying final month's dues along with 30% of the remaining month's dues owed for that contractual term. All fees are due upon notice of cancellation.
End of Term/MTM: Not Renewing. Term date is based on agreement contract. (30 days notice required)
